Benefits
HEALTH
INSURANCE- We offer a choice of First Priority Health (HMO or POS)
and Geisinger PPO. For those employees located in the "field" we
offer the Access Care II PPO plan with an HRA account. Penn Millers
pays 65% of the dependent and 75% of the individual premium portion -
the employee is then responsible for the difference.
Eligibility: Full time employees, 1st of the month after 60
days of employment.
DENTAL
– United Concordia provides 100% coverage for preventative
services, 80% for basic restorative services and 50% for major
restorative services. Penn Millers pays 50% of employee coverage,
employee is responsible for the difference and for the cost
of dependent coverage.
Eligibility: Full time employees, 1st of the month after 60
days of employment.
VISION
– Davis Vision provides coverage for eye examinations
and refractions, contact lens prescription and fittings and
eyeglass frames and lenses. Penn Millers pays 50% of employee
coverage, employee is responsible for the difference and for
the cost of dependent coverage.
Eligibility – Full time employees, 1st of the month
after 60 days of employment.
GROUP
TERM LIFE & ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE
– through GE Financial Assurance, life insurance and
accidental death & dismemberment coverage equal to 1 ½
times annual salary rounded to the next $1,000.00. Penn Millers
pays the full cost for this program.
Eligibility – Full time employees, 1st of the month
after 60 days of employment.
LONG
TERM DISABILITY - through GE Financial Assurance, a disability
benefit of 60% of base monthly salary after 90 days of disability.
Penn Millers pays the full cost for this program.
Eligibility – Full time employees, 1st of the month
after 60 days of employment.
SALARY
CONTINUANCE – Penn Millers provides a partial
salary continuance for employees who are away from work for
an extended period of time due to an illness or disability.
This program helps to fill the gap between the time when an
employee is first out of work and the time when they are eligible
for long term disability. This benefit is fully funded by
Penn Millers.
Eligibility – Full time employees with more than one
year of service.
Time Off Benefits
PERSONAL
TIME OFF (PTO) – Paid time off away from work to be
used for reasons such as vacation, illness, illness of a family
member or personal business. The number of days is based upon
length of service and ranges from 19 – 30 days per year.
Eligibility: Full time and regular part time employees
PAID
HOLIDAYS – Penn Millers observes the following company-paid
holidays: New Years Day / Memorial Day / Independence Day
/ Labor Day / Thanksgiving Day/ Day after Thanksgiving / Christmas
Day Eligibility – Full time and regular part time employees.
BEREAVEMENT LEAVE – up to three consecutive paid days
off in the event of the death of a parent, parent-in-law,
spouse, brother, sister, child or relative residing in the
employee’s home. One paid day will be granted to attend
the funeral of an employee’s grandparent, aunt, uncle,
niece or nephew, brother-in-law or sister-in-law.
Eligibility: Full time and regular part time employees.
JURY DUTY - paid time off when summoned to serve on Jury Duty.
Eligibility – Full time and regular part time employees.
MILITARY LEAVE –In addition to the rights and benefits provided to
employees taking extended Military Leave, eligible employees who must
be absent from their job for a period of not more than 10 working days each
year in order to participate in temporary military duty are entitled to as
many as ten days unpaid military leave. All benefits will continue during an
employee’s temporary leave.
Eligibility – Full time and regular part time employees.
FAMILY AND MEDICAL LEAVE (FMLA) – employees may be granted
up to 12 weeks of job protected unpaid leave in a 12-month
period for the birth or adoption of a child, a serious health
condition of their spouse, child or parent, or their own serious
health condition.Eligibility – Employed for at least
12 consecutive months and worked at least 1,250 hours during
those last 12 months.

Benefits
401(K)
PROGRAM – through MassMutual we offer a program that allows employees to
contribute a percentage of pay, up to the annual IRS limit, to a 401(K) program.
Penn Millers will match $0.50 on each dollar contributed up to the first 6% of
salary.
Eligibility – Full time and regular part time employees
PENSION
PLAN – a defined benefit program that provides a retirement
benefit the amount of which is determined based upon compensation
and length of service. There is a five-year vesting plan under
this program. Penn Millers pays the full cost of this program.Eligibility
– enrollment in the plan is January 1st of each year.
To be enrolled, an employee must be age 20 ½ and have
completed six months service.
